CIRS Medical Treatment-
Full Medical Intake | 85-105 minutes of medical care | $899-$1,119
Appointment includes 30 minutes of medical review prior to visit, a 40 to 60-minute telehealth visit based on case and complexity, and 15 minutes of post-visit plan creation & charting*
We require a $250 nonrefundable deposit to book your Initial Medical Evaluation.
Initial Medical Follow-up | 60 minutes of medical care | $660
Appointment includes 10 minutes of medical review prior to visit, a 40-minute telehealth visit, and 10 minutes of post-visit plan creation & charting*
A one-time personalized report on labs and diagnostic criteria is available prior to visit at a flat rate of $125. This enables your pre-visit review to maximize time spent with Dr. Sparks.
Subsequent Full Medical Follow-up | 60 minutes of medical care | $550

Subsequent Focus Medical Follow-up | 30 minutes of medical care | $275
Appointment includes 5 minutes of medical review prior to visit, a 20-minute telehealth visit, and 5 minutes of post-visit plan creation & charting*
If next steps in your treatment may be covered in a Focus Session, Dr. Sparks will let you know.
*We assess a surcharge of 8.44% on all medical fees to cover GRT and service fees incurred.
*Care extending beyond designated time frames above will be prorated at Dr. Sparks’ hourly medical rate of $660/hour.
Advanced testing such as NeuroQuant brain MRI and GENIE require advance medical review, pro-rated at the above rate.
Some care can be delivered asynchronously without compromising quality of care. We will make you aware if and when this is appropriate. We will provide you with a fee schedule for this care.
CIRS Proficiency Partner Coaching-
